건강보험 가입 재가 장애인의 미충족 의료 경험에 영향을 미치는 요인

Abstract

이 연구의 목적은, 장애인의 의료접근성 강화를 위한 건강보험 정책수립에 유용한 실증적 근거를 제공하는데 있다. 이를 위해 2008년 장애인 실태조사 자료에 포함된 만18세 이상 재가 장애인 중 건강보험에 가입된 4,520명을 대상으로 미충족 의료 경험 현황과 원인을 분석하였다. 주관적 욕구, 필요 재활보조기구 소지, 그리고 건강검진 수검에서의 미충족에 영향을 미치는 요인에 대하여 로지스틱 회귀분석을 시행한 결과, 교육수준이나 소득은 통계적으로 유의하지 않은 반면 배우자 유무, 일상생활 동작 도움 필요, 만성질환 유병 상태 등 건강관련 요인과 이동성 제약 요소, 건강보험 급여 내용 인지여부가 장애인들의 미충족 의료 경험에 유의한 영향을 미치는 것으로 나타났다. 이러한 연구결과는, 건강보험에 가입된 장애인의 미충족 의료 해소를 위해서는 방문돌봄, 교통수단 및 급여정보 제공 등 비경제적 접근장벽을 완화해주는 지원이 필요함을 시사한다.

The purpose of this study is to examine determinants of unmet healthcare needs among the community-dwelling Korean adults with disabilities. Needs are defined as subjective demand, the possession of necessary rehabilitation equipment, and health screening service. Logistic regression estimations using data from the 2008 Survey of People with Disabilities find that health-related factors such as prolonged chronic illness and limitations in ADLs and IADLs are statistically significant factors whereas educational level and income are not. Results further show that non-monetary factors such as the presence of informal caregivers, physical mobilitiy, and knowledge of the insurance benefit are important to ensure adequate access to necessary care among people with disabilities.
